Alumni Achievement Award Nomination Form

The Alumni Achievement Award is given to a graduate who displays great accomplishment(s) in one or more of the following areas: professional achievement, community volunteer service, and humanitarian efforts. Through these accomplishments, these alumni bring recognition to themselves and to Holy Family University.

Nominations must be received by February 15. In addition to being recognized on campus and in various other promotions, winner(s) will receive two complimentary tickets to Reunion 2022, where they will be formally recognized by their peers and the HFU community.

To nominate a Holy Family alumnus or alumna for this Award, please complete the below form to the best of your ability. The Selection Committee will judge nominees solely based on the information and supporting documents provided in this nomination form.
