COVID-19 Self-Screener

All members of the Holy Family University community, as well as visitors, are encouraged to complete the daily COVID-19 self-screener. Select the appropriate screener, and follow the guidance based on your responses. Thank you for doing your part to help Holy Family University to #StaySafe!  

COVID Guidelines

  • Quarantine and self-isolation are not required, unless you are exhibiting COVID-19 symptoms.
     
  • If you are experiencing COVID-19 symptoms, please notify your supervisor that you will not be reporting to work, isolate yourself, and contact your medical professional for COVID-19 testing.
     
  • Follow all University mitigation efforts, including mask wearing in all indoor and outdoor spaces.
     
  • Follow any applicable federal, state, local laws or regulations.

In the past 48-hours, have you been in close proximity to anyone who has tested positive for COVID?
Other than in the work environment while wearing appropriate PPE, in the past 14 days, have you been in close proximity (six feet or less) to anyone who has tested positive for COVID?
Have you been tested for COVID and are waiting to receive test results?
Have you tested positive for COVID, or are you presumptively positive for COVID, based on your health care provider’s assessment or your symptoms?

If you answered Yes to any of the above questions, notify your supervisor that you will not be reporting to work, isolate yourself and contact a medical professional regarding advice on testing and next steps
