In compliance with the Pennsylvania Department of Education (PDE), Holy Family University will report successful completion of courses taken to satisfy ACT 48 regulations. The report will be sent to PDE and the professional educator (the student). Records of completion of one to six credits at Holy Family will become part of the student's permanent transcript. To request that your course completion (1-6 credits) be reported in compliance with ACT 48, you must complete this form.
